OFFICIAL RULES & DISCLOSURES

This is a cash 50/50 raffle benefiting FACES: The National Craniofacial Association.

• Prize: The winner receives 50% of total ticket sales; the remaining 50% benefits FACES programs and services. There will be only one winner. FACES will not split or divide the prize among multiple people — the full prize amount will be paid to and reported under the single winning ticket holder.
   

• You do not need to be present to win. The winner will be notified by phone call. FACES will make at least 3 attempts to reach the winner; if the winner cannot be reached within 14 days of the drawing, the prize will be forfeited and a new winner will be drawn. FACES is not responsible for illegible handwriting on ticket stubs — if your name and/or phone number cannot be read, your ticket will be eliminated from the drawing. Please write neatly.
   

• Eligibility: Per Tennessee Charitable Gaming Law, tickets may not be purchased by any FACES board member, officer, or employee; any immediate family member residing in the same household as a board member, officer, or employee (defined as spouse, child, stepchild, brother, sister, son-in-law, daughter-in-law, parent, or grandparent); or any vendor (including their employees, agents, or subcontractors) providing printing, telephone, or other gaming-related supplies or services for this event.
   

• Drawing location: The official drawing will be held at the FACES office in Hixson, TN. The winner announcement event will be held at Cesareo's Pizzeria in Rossville, Georgia.
   

• Tax reporting: This prize is reportable income. FACES will report the full winnings to the IRS under the sole winning ticket holder, who will be required to complete a Form W-9 to claim the prize.
